We have a long tradition of training, our first Registrar starting in the 1960s. There are two trainers, Dr John Tan and Dr Philippa Dickins. Dr Tan is the main trainer doing tutorials and joint consultations while Dr Dickins does one session of random case analysis a week. The Registrars are also encouraged to learn from the other Doctors in the Surgery and other members of the Primary Health Care team.
The weekly time table includes a 1 ½ hour tutorial, 1 hour of joint consultations, 2 hours of random case analysis and 1 afternoon of private study. Our monthly clinical education meetings include Random Case Analysis, Journal Club and both In-House and Consultant teaching sessions. We also have annual CPR training and occasional evening speaker meetings.
We encourage the use of study leave for the MRCGP and other courses including Minor Surgery and Family Planning. All Registrars attend the Poole Day Release Course on Wednesdays. When the course is not running, they use the afternoons for attending Outpatient clinics and other educational activities.
Initially, Registrars have plenty of supervision and start with 20 minute appointments. Towards the end of the year, we prepare them to become partners or assistants in General Practice with 10 minute appointments. More responsibility is given, including visits and looking through the mail and pathology links.
About the Practice
The Quarter Jack Surgery is a large group practice of 7 partners and 3 assistants serving the ancient market town of Wimborne Minster in Dorset. We are situated 6 miles north of Poole with its magnificent harbour and sailing opportunities, on the edge of beautiful rural Dorset and close to the beaches of Bournemouth and Poole. Poole also has a thriving Arts Centre which is home of the Bournemouth Symphony Orchestra.
We serve a population of approximately 13,000 patients. We are fortunate that most are in employment and there is little social deprivation. The majority live in the urban locations of Wimborne and Colehill but slightly more than 2,000 live in the villages north of Wimborne in rural locations and we provide a dispensing service for them.
A very exciting aspect of our practice is our link with Wimborne's community hospital. It is situated on the edge of the town not far from the practice. We look after the minor casualty department which functions from 8.00 a.m. to 9.00 p.m. We provide post-op care for orthopaedic, gynaecology, urology and general surgery; we have access to GP beds to which we can admit our patients acutely; and we have access to two palliative care beds where we can care for our terminally ill patients. The hospital also allows us rapid access to X-ray, ultrasound, physiotherapy and occupational therapy. Many visiting consultants provide out patient clinics in Wimborne Hospital, which allows us close liaison with them in most specialities.
We practice from attractive modern purpose-built premises and the registrar has their own consulting room. We have an up-to-date library, video facilities and are linked to the NHS Net. We are fully computerised in our consultations and use pathology links and have our letters scanned into the computer system. We have always achieved 100% of our QOF points each year.
Special Interests within the practice
Many of our doctors have special interests and are presently or have recently been GPSIs in their subject. We encourage cross referrals within the surgery to enhance our knowledge and reduce our referral rate. The following specialties are represented: Dermatology, Orthopaedics, Rheumatology, Cardiology, Elderly Medicine, Breast Disease, General Surgery, Urology and Gastroenterology. We have Nurse Practitioners offering acute same-day appointments and Specialist Nurses in Cardiology, Asthma and Family Planning.
